The Health Trust provides a variety of services to low-income Santa Clara County residents living with HIV/AIDS. These services include, but are not limited to, medical and non-medical case management, care coordination, and food assistance.

The Health Trust provides a variety of services to low-income Santa Clara County residents living with HIV/AIDS. These services include, but are not limited to, medical and non-medical case management, care coordination, and food assistance.

Non-Medical Case Management
Eligible clients receiving non-medical case management are provided the following support:

Comprehensive Assessments every 90-120 days
Service Plans developed and reevaluated every 90-120 days
Completion of AIDS Drug Assistance Program (ADAP) enrollment, Ryan White certification
Referrals to community resources, including The Health Trust Jerry Larson FOODBasket
Support in maintaining medical care
Medical Case Management
Eligible clients receiving non-medical case management are provided the following support:

Comprehensive assessments with client every 6 months
Service Plans developed and reevaluated with client every 6 months
Completion of AIDS Drug Assistance Program (ADAP) enrollment, Ryan White certification
Intensive case management and coordination of HIV medical care and referrals to community services
Assistance and support with disability income applications, benefits, In-Home Support Services (IHSS)
Medical Case Managers regularly attend medical visits with clients
Harm reduction education and health education
Support towards self-sufficiency

RN Case Management
Eligible clients receiving non-medical case management are provided the following support:

Registered Nurses evaluate client and make referrals for necessary services in their home such as homemaker services, attendant care, Meals on Wheels, durable medical equipment, etc.
Provide intensive assistance with medication management, including med trays and pill counts
Coordinate medical care and assure medical needs are met
Ongoing health education and disease management support

Select HIV/AIDS Services clients are granted access to nutritious and fresh foods at our food pantry twice per month. Grocery and Meals on Wheels delivery is also available for eligible clients.

Project START Plus
A Centers for Disease Control and Prevention behavioral intervention, this program is an individual-level intervention for individuals living with HIV preparing for release from a correctional facility and returning to the community. Clients receive one-on-one support with linkage to care and harm reduction planning. Our Project START Plus staff work inside Elmwood and the Main Jail with incarcerated clients and continue to support them post-release, working closely with Custody Health and PACE providers.

Housing for Health This program provides a permanent housing subsidy to Santa Clara County residents living with HIV who are low-income and homeless or at risk for homelessness. Participants receive case management from a Housing Services case manager and ongoing support to maintain their housing.
